Nurse Navigator at Baylor Scott & White Surgical Hospital Fort Worth:
The Nurse Navigator is responsible for providing patient guidance throughout the continuum of care for Total Joint and Spine service line patients. This professional will serve as the education lead for patients preparing for surgery. The Nurse Navigator will provide resources and coordination of care for patients and their families. This professional will work closely with the interdisciplinary team to ensure a smooth transition between phases of care. The Clinical Nurse Coordinator works with the Director of Quality & Medical Staff Services in meeting unit/department goals.
- Assist in the development of education for patients and staff, to align with the performance measures for the Total Hip & Knee Replacement and Advanced Spine Surgery certifications.
- Round on the total joint and spine patients post-operatively to anticipate discharge needs.
- Interacts and facilitates care given by and communication between all care providers (nursing, physicians, physical therapist, pharmacy, radiology, case management, social work, etc.).
- Facilitates patient and family education by developing appropriate education programs, resources and tools to address needs to include comprehensive preparation class schedule for both total joint and spine programs as well as other programs developed by facility leadership.
- Evaluates, develops and updates patient education material content for accuracy, and literacy level every two years or as needed.
- Demonstrates the ability to gain the patient’s understanding and acceptance of the Total Joint and Spine processes through patient feedback and data collection.
- Follows up with patients (rounding and phone calls) and providers in a timely manner to discuss the patient’s response to treatment, as well as obtain progress reports in conjunction with Centers of Excellence Programs.
Education
·Graduate of an accredited school of nursing
Licensure/Certification
·Current Texas RN License
·Current BLS
Specific Job Experience
·Minimum five (5) years’ experience as an RN in a hospital Medical/Surgical or ICU Unit
