Fresno Surgical Hospital is known for excellence in surgical care delivery and patient satisfaction. Our goal is to provide a high quality of care through compassion to all we serve by creating a supportive environment for patients, clinical staff and employees.

This is a dual position: 

The Joint Coach will assume responsibility and accountability for the entire care pathway of the total joint patient. Optimization of the program will be achieved by facilitation care for patients by serving as a liaison throughout the continuum of care. This is achieved by anticipating and addressing each patient’s needs before, during and after surgery. There is ongoing collaboration to create a team approach with the medical team, including the surgeon, anesthesiologist, Pre-op, PACU, OR staff, PT provider, leadership and the key members from the surgeon’s office. In addition to the day-to-day activities required, the position requires the Coach to participate in data collection, analysis and reporting of clinical outcomes and patient satisfaction.

The Hospital Educator is responsible for developing, implementing, conducting and evaluating in-service programs, orientation and continuing education for other nurses and other staff; establishes educational policies and procedures; acts as a resource to staff and management; participates in performance improvement actives. Serves on committees. 

Salary Range: $45.00 - $66.96 (Depending on Experience)

Hours: 8 M-F


Required Skills:

Minimum Education and Experience

  • Completion of an accredited registered nursing program.
  • Bachelor's Degree in Nursing rom a four year college or university.
  • Two or more years of clinical/education experience preferred. 
  • MSN or Master's Degree in related field and/or graduation from advanced nursing program preferred. 

Required Licenses and Certifications

  • Current California Registered Nursing License
  • BLS Certificate required
  • ACLS Certificate preferred

  • Must be passionate about patient education.
  • Requires the expertise to apply sound judgement to ensure optimal patient care.
  • Strong communication skills are required to instruct patients, family members and staff.
  • Must be self-directed with strong interpersonal and organizational skills.
  • Computer literacy with MS Office Word and excel is required.
  • Able to plan and prioritize with frequent interruptions.
  • Ability to provide exceptional customer service.
  • Demonstrates the ability to work as a steam member and to communicate directly and professionally.
  • Ability to utilize/apply the general and specialized principles, practices, techniques and methods of utilization review/management, care coordination, transfer coordination, discharge planning or case management. Knowledge of relevant equipment, systems, change theory, policies, procedures, and strategies to promote effective quality outcomes. Demonstrates the ability to work as a team member and to communicate directly and professionally. Is flexible, reliable, productive, and patient oriented.
  • Knowledge of adult learning methods.
  • Able to adapt in response to new information, shifting priorities, changing conditions, or unexpected obstacles.
  • Ability to train, and effectively communicate with customers including listening, asking relevant questions, giving appropriate guidance and feedback.
  • Willingness and understanding of completing time critical projects/tasks.

Essential Functions

  • Leads and directs the Total Joint Coaching programs to include patient coaching, reporting, metrics, physicians relations, and employee management.
  • Works closely with multidisciplinary team to develop and implement the patient’s care plan through assessment of the patient’s physical, educational, and physical, educational, and psychosocial needs.
  • Serves as an essential link between patients and all other healthcare providers.
  • Serve as a resource to the patient through their continuum of care including prior to surgery and extending after discharge.
  • Facilitates pre-operative education and training for the patient and care partner.
  • Facilitates scheduled follow-up calls with patients to promote compliance with discharge instructions and risk mitigation calls to avoid preventable ER visits/admissions, confirm physical therapy and post-op visits to physicians. This will include obtaining data for Patient Reported Outcomes at the pre-op visit, 6 month, and 1 year follow-up.
  • Models the Expected Behaviors for Patient Centered Care Pathway and is passionate about the patient experience.
  • Ensures all necessary follow-up/action items are completed, reported and documented in order to close the loop.
  • Participates in QAPI activities, PRO reporting to home office, and completing EDGE requirements. This may require coordination with QAPI program coordinator.
  • Requires flexibility in job sharing responsibilities associated with other specified job description.
  • In partnership with departmental educators, leadership, and Unit Based Teams conducts a needs assessments of each department annually.
  • Develops and directs orientation programs, preceptor programs, and inservices in collaboration with department leadership and/or departmental educators.
  • Sets expectations and guidelines for departmental educators in collaboration with nursing leadership to standardize processes for clinical departments.
  • Incorporates frontline team in developing in-services and teaching.
  • Implement and supervise the nurse preceptor program; supervises all students and new graduates in the clinical areas in coordination with Clinical Managers/Leads.
  • Lead RQI Implementation and coordinate scheduling of frontline team.
  • Lead monthly educational meetings with department educators and/or leadership.
  • Lead preceptor meetings at least 2 times per year.
  • Act as a departmental educator as directed.
  • Plans quarterly mock codes and develops and implements Mock Code Team with frontline and leadership involvement.
  • Assists with the coordination of performance improvement and continuous quality improvement activities.
  • Works with the CNO to develop a budget for educational programs.
  • Ability to grant proposals and manage grant requirements.
  • Assist with organization wide education projects.
  • Maintains and leads educators to maintain regulatory requirements including all state, federal and joint commission regulations.
  • Provides educational opportunities that benefit nurses at all stages of their career development.
  • Develops evidence-based educational materials.
  • Supports the integration of research into practice.
  • In partnership with departmental leadership, participates in the selection of internal candidates for departmental educator selection.
