Under the direction of the System Director of Pharmacy, the Pharmacist is directly responsible for the day-to-day functional duties of the Department of Pharmacy in accordance with the hospital’s stated philosophies, goals and objectives to ensure the consistent provision of high quality patient care.Always works as a team member within the entire facility. Performs other duties as assigned.
Essential Functions:
- Interprets medication orders (verbal and written) and transcribes to computerized patient medication profiles accurately.Maintains accurate, complete patient medication profiles.
- Monitors medication therapy regimens for contraindications, interactions, allergies and appropriateness of medication and dose.
- Ensures safe, appropriate, cost effective medication therapies for patients according to established policies, procedures, and protocols.
- Supervises and directs daily activities of pharmacy technicians.
- Provides accurate, adequate, and timely medication information to the professional staff.
- Performs periodic audits of medication administration as instructed by System Pharmacy Director.
- Completes and documents inspections of all assigned medication storage areas at least monthly.
- Satisfactorily demonstrates the knowledge, skills and abilities to perform the duties outlined in this job description as well as continually demonstrates competency in performing the job duties.
- Satisfactorily demonstrates the knowledge, skills and abilities to perform the duties for Antibiotic Stewardship.
Qualifications:
- Licensure as a Registered Pharmacist by the Oklahoma State Board of Pharmacy required.
- Certificate as a preceptor by the Oklahoma State Board of Pharmacy after one year of licensure preferred.
- Education: Undergraduate pharmacy degree (Bachelor of Science) or Pharm.D preferred.
- One (1) year experience in a hospital pharmacy preferred.
- Must be highly proficient in the clinical skills regarding patient assessment and pharmaceutical care.
- AHA BLS-HCP within 30 days.
